           WORKERS’ COMPENSATION “NOTICE ONLY” CLAIMS
 
Notice Only Claims are identified as:
1)      Employee sustains a work-related injury/illness and does not want to file a workers’ compensation claim nor seek medical treatment.
 
Procedures for reporting a “Notice Only” Workers’ Compensation Claim is as follows:
 
1.       Notify your supervisor immediately.
 
2.      Complete Manteca Unified School District “Employee’s Report of Work Injury/Illness” form. 
 
·        On the form you will be asked if you require medical attention at this time.  Please mark “no” and sign.
 
·         If you need medical attention at a later date, you must contact Risk Management.  An appointment will be scheduled to have you come into Risk Management and complete the Workers’ Compensation Claim Form, (DWC 1).  You will be required to go to the doctor’s at the time you file the (DWC 1). 
 
  1. Return the completed form to your Office Manager.  This form will remain on file in Risk Management and will not be submitted to our workers’ compensation insurance carrier.   A claim will not be established until at which time you complete a Workers’ Compensation Claim Form, (DWC 1) and you receive medical treatment.
