How to Register your Student
Complete the First Day Packet registration forms beginning on page 18.

- Internet Consent Form
- Student Residency Questionnaire
- Title VI Indian Eligibility Form
- MUSD Registration Form
- Authorization for Medication Administration Form
Bring the completed forms to your school site after July 23rd, along with:
- Current proof of residency
(rental or utility contract, property tax receipt, pay stub, voter registration card, or correspondence from a governmental agency)
- Child's Immunization record
It has a map of all school boundaries, discusses how school placement is determined and gives you a glimpse of who we are as a school district.
