If your student is currently enrolled in a MUSD preschool or TK program for the 2020-2021 school year and will be eligible for kindergarten for the 2021-2022 school year, your school of residence will contact you in March to schedule an in-person reenrollment appointment. This appointment will take place on or after April 7, 2021.
When you arrive for your in-person appointment, you must bring the following documents for verification:
- Completed Registration Form (En Espanol)
- Proof of residency
- Immunization records (View a list of required immunizations - en Espanol)
- Child’s birth certificate
- Oral Health Assessment form (en Espanol)
- Physician’s Report form
- Authorization for Medication Administration (if applicable)
- Parent Rights and Responsibilities (En Espanol)*
*Please read and print the document, and bring it with you to your in-person re-enrollment appointment. If you do not have access to a printer, please read over the document online and notify the attendance clerk or registrar at your school that you will need assistance printing the consent form during your appointment.
Please note:If you are registering your child for Kindergarten, you will be prompted to upload an Oral Health Assessment Form (which confirms the child has been seen by a dentist) (En Espanol) and Physician’s Report Form (which confirms the child has received a physical by a doctor) as part of registration.
To make the registration process faster, please ensure your child’s shots are up to date, the child has been seen by a dentist, and a physical has been completed by a doctor.
The Oral Health Assessment must take place between August 2020 and the first day of school in August 2021.
The child’s physical must occur after February 8, 2021 and prior to the first day of school in August 2021.If your child has yet to be seen by a dentist or doctor, an upcoming appointment card may be brought in for your in-person appointment.
