Work Permit Application Form

SUMMER WORK PERMIT SCHEDULE 2023

During the School Year Instructions:

Step 1 - Download form from the link provided and print it out.
             ( If you do not have access to a printer, you can come to the front office to pick one up.)

Step 2 - Complete the Minor’s Information section of the form. Be sure to include your social
             Security number and parent/guardian signature

Step3 -  Take the application to your employer for their section to be completed and signed.

Once your application is filled out completely, please drop it off in the office at Lathrop High to be processed. If you have any questions, please contact the office. When it is ready to be picked up, you will be notified by teams or by email. 

** Please allow 24-48hrs. for a permit to be processed. Incomplete forms cannot be processed and will be returned.
