Student School Benefit Form

The Student & School Benefits Form (SSBF) is a form that we ask all parents to fill out once a year. This form asks a few short questions about your child and household. The information you provide is kept private and is not reported in connection with your name or family.

 

Why is This Form Important? 

The information gathered from this form affects how state and federal funds are distributed to your child’s school, as school needs are determined by the needs of students. By completing it, you are helping ensure your child’s school receives the necessary resources for your child and their classmates, such as teachers and support staff, classroom supplies, technology, extracurricular activities, and additional student support services.

 

How Do You Complete the SSBF? 

You can complete the SSBF in one of the following ways:

  1. Complete the SSBF in your Q Parent Connect account by clicking the “Links” button under the Main Menu and then clicking the “Student & School Benefits” link.
  2. Download the digital form on this webpage, fill it out, and email it to community@musd.net.
  3. Print the digital form from this webpage, fill it out, and return it to your child’s school or the MUSD District Office.
  4. Call your child’s school office and request a staff member complete the form on your behalf.

 

How Could This Form Benefit Your Child and Family?  

By completing the form, you may qualify for many significant benefits, including reduced PG&E and Comcast internet bills and waived college application and SAT/ACT testing fees. If your household is low-income and you submit your form before August 31, 2024, you may qualify for the 2024 Summer S-EBT card.
