Register for the Current School Year 2023/24

Welcome to MUSD! We are so happy to have you join our school community.

To begin, please use the School Site Locator tool to look up your child’s school of residence, then access the Q portal below to complete pre-enrollment. Once you click on the Q portal button, you will fill out pre-enrollment information and scan and upload required documents.

Once you have submitted the required information and documents in Q, you will be alerted that you have successfully pre-enrolled your student and will receive a confirmation email. Your school of residence will contact you to set up an in-person registration appointment to verify required documents.  

Documents that must be brought during your registration appointment include:  

• Immunization records  (View a list of required vaccinations | En Espanol)
• Child’s birth certificate   
• Authorization for Medication Administration (if applicable)

If you are pre-enrolling your child for Kindergarten, you will be prompted to upload an Oral Health Assessment Form (which confirms the child has been seen by a dentist) (En Espanol) and Physician’s Report Form (which confirms the child has received a physical by a doctor) as part of pre-enrollment in Q. To make the registration process faster, please ensure your child’s shots are up to date, the child has been seen by a dentist, and a physical has been completed by a doctor. The Oral Health Assessment must take place between August 2021 and the first day of school in August 2022.  The child’s physical must occur after February 14, 2022 and prior to the first day of school in August 2022. If your child has yet to be seen by a dentist or doctor, an upcoming appointment card may be uploaded during pre-enrollment.  View a list of required vaccinations (En Espanol).
