If you are pre-enrolling your child for Kindergarten or First (1st) Grade, you will be prompted to upload an Oral Health Assessment Form (which confirms the child has been seen by a dentist) (En Espanol) and Physician’s Report Form (which confirms the child has received a physical by a doctor) as part of pre-enrollment in Q. To make the registration process faster, please ensure your child’s shots are up to date, the child has been seen by a dentist, and a physical has been completed by a doctor. The Oral Health Assessment must take place between August 2023 and the first day of school in August 2024. The child’s physical must occur after February 14, 2024, and prior to the first day of school in August 2024. If your child has yet to be seen by a dentist or doctor, an upcoming appointment card may be uploaded during pre-enrollment. View a list of required vaccinations (En Espanol).
