If your child is currently enrolled in MUSD in a TK or preschool special education program OR was previously enrolled in MUSD in any grade level, please follow the steps below.

Step 1: Find your school

First, use the School Site Locator tool to look up your child’s school of residence. The school of residence is the school assigned to a student based on their home address! Please note: this may not be the same school your child attended previously for TK.

Step 2: In-Person Registration Appointment

Contact your school of residence to schedule an in-person registration appointment.

Please bring the following documents to your registration appointment:

• Proof of residency 
• Immunization records (En Espanol)
• Child’s birth certificate

The following documents will be completed during your registration appointment. You do not need to print and complete these in advance, however you may if you would like. 

• Completed Registration Form (En Espanol)

• Authorization for Medication Administration (if applicable)

• Parent Rights and Responsibilities (En Espanol)*

• Overflow letter English |(En Espanol)

• Residency Questionnaire 

NOTE: If you are enrolling your child in Kindergarten or First (1st) Grade, you will be prompted to fill out an Oral Health Assessment Form (which confirms the child has been seen by a dentist) (En Espanol) as part of enrollment process. 

To make the registration process faster, please ensure your child’s shots are up to date and your child has been seen by a dentist. If your child has yet to be seen by a dentist, an upcoming appointment card may be valid.  View a list of required vaccinations (En Espanol).
